You just got to collect these materials: 5 lucid gems, 5 power gems, 5 thunder gems, 3 mystery goos, and 3 gales.
Once you have these, the moogles should automaticly make it for you.
As far as Sephiroth, don't let him pull off heartless angle, attack him to stop it. Also have your stock full of mega elixers and such, don't be afraid to use em! (Sephiroth took me 2 months or more to beat, lol)
