1) Time would go either at the very beginning of the sentence or right before the object.
By adjectives, I take it to mean those modifying ねこ in this sample sentence. Then they should be placed right in front of ねこ.
Regarding questions #2 + #3, I just don't know anything about JLPT for obvious reasons.
