In reality - Those
between 40 and 74 who weigh over the "limit" are informed that they are overweight, and encouraged to lose the weight. If there are already health issues involved (high blood pressure, high cholesterol, etc) then they are scheduled for a followup with a weight counselor, and given support to lose weight and improve their health.
There is no fining involved. It is not "against the law" to be overweight. They are just offering free support and counseling to help people actually lose extra weight and be healthier.
