Your issue is not that you are transexual or that you are black, it is as above that until now, Japan has only ever accepted a handful of foreign nurses.
Currently there is a programme to bring over Indonesian and Filipino nurses to work in geriatric care, with the condition that they have to pass the Japanese registration exam within 2 or 3 years, or go back to their home countries. As above, they reported in the news recently that of over 200 nurses, only 3 had passed the exam after more than a year. Their nursing skills are fine, but as the experience of the Asian nurses shows, it is very hard to reach that level of Japanese reading and writing ability, even if you live here.
Unless your Japanese language skills are similar to a native Japanese speaker's (this usually takes many years of study), I am afraid that your goal of becoming a nurse here is unrealistic. Even if your Japanese was that good, so far they are only allowing nurses in on special programmes from particular countries as above, and Brazil isn't one of them.
