In regards to the 'modern mentality' towards nuclear weapons, it comes down to Mutually Assured Destruction, or M.A.D. I think it's something along the lines that neither side will deploy nuclear, biological or chemical weapons if both sides has them. Or an ally of the attacked party has them.
Or something like that. Look it up if you want to find out more about M.A.D., because I can't be bothered!

Its true... MAD works on a large scale between global superpowers
On a smaller scale though it gets a bit tricky.
2 small or medium powers that have nukes, fighting a contained, regional conflict wont have MAD on their mind.
