On a more serious note, I would have guessed it meant people with pacemakers or similar, so yeah heart conditions.
Although it would also be nice if people gave up a seat for someone feeling emotional!

A seat for manic depressives? Suicidal people? hmm~ might not catch on.
You often see signs near the priority seat one asking people to turn off their phones if they're sat close to them as well as give up a seat. Apparently some mobile phone frequencies can screw around with some pacemakers. 8/
