I think 助ける carries more a connotation of "assistance needed" than the other does.
I think 手伝う carries more a connotation of "assistance optional" than the other does.
If you're in trouble, it's 助けてくれ! you yell. If you helped your brother move to a new apartment, you 手伝った him. Even look at the kanji of 手伝う: you are transferring a hand.
