In the 1st box, put your name. In the 2d box, put a Katakana transliteration of it if you wish. I highly doubt they're going to reject your order because you didn't fill it out, and if they do, just re-enter your info with the furigana filled in.

Just to make you understand. Furigana is a set of kana which appears on top (or on the said depending on wheather written western or eastern style) of a set of kanji. These characters are there to tell you how to read that kanji.
If you live in a western country I doubt you need to feel the second box. Even if you were Japanese it might create problems to have a package sent to you with kanji in the address. 暗闇の中 歩くしかねぇ everything’s gonna be okay 恐れることねぇ 辛い時こそ胸を張れ |

I think the furigana requirement is probably so if the company ever has to call you, they'll know how to read your name. It's unfortunate that we don't have this in English, because I get calls asking for "geets" and "gotez" all the time.
