Tax Return/Withholding Statement (Gensen Choshu-hyo)
 

I'm renewing my visa and I need my tax withholding/return statement that my company gave me in January but of coarse . . . I can't seem to find it. Has this happened to anyone else? What should I do?

You'll need your Alien card, employment certificate, your employment contract, tax statement, the application form, passport and 4,000 yen
