You need to go to the district ward (区役所 kuyakusho), the one where your card was first registered unless you moved (and filed the paperwork with the ward to register the move)

Umm, half the "suburbs" around here are 町. You don`t have to live out in the middle of nowhere to live in a 町. Tokyo even has some 町.
